Spontaneous Gastric Perforation in the Newborn


Abstract
Spontaneous gastric perforation is a frequent cause of pneumoperitoneum in the neonatal period.
Castleton and Hatch in 1958 suggested that the term be used when the following criteria are met, (1) newborn, (2) no, evidence of ulcer, (3) no evidence of perforation due to nasogastric tube, oxygen or trauma etc, (4) no evidence of distal obstruction.
Possible etiology is discussed. Neonatal asphyxia was the most common predisposing cause (63%)
The first clinical sign is abdominal distension with or without respiratory distress. Roent,,genograms of abdomen are most-helpful in making a positive diagnose, surgical repair in two layer after debridement is the treatment of choice.
The mortality was 27% in the, cases operated since 1970, a significant improvement as compared to a mortality rate of 62% for the cases operated before 1970. Recently we experienced in. 5 day old male patient, under the diagnosis of hollow viscus perforation, emergencelaparotomy was done, when diagnosed as spontaneous gastric-perforation. With the surgical treatment of debridement and 2-layer repair, post-operative recovery was uneventful.
We report our own one case with the review of literature.
